Scheduling Information

All appointment requests are requests. I will do my best to honor requests but they will be honored in the order in which they are received. I will reach out to you with the provided information if any changes are needed.

Coastal Patients

Select a time and schedule below. All appointments will be defaulted to virtual video visits unless specified in the notes on the appointment request.

Headway Patients

Select a time and schedule below. Please indicate you are from Headway in the “What is the reason for your visit” section. All appointments will be defaulted to virtual video visits unless specified in the notes on the appointment request.

New Patients

New patient appointments will require completion of new patient paperwork prior to scheduling. For consult calls, no paperwork is necessary.
New patient visits are scheduled for 1 hour. You will need to book two slots, back-to-back for a new patient visit. I strongly recommend a consult call prior to booking a new-patient appointment.
